This Annual Block Fee is an annual payment that covers many health services that are not traditionally covered under OHIP. These uninsured services include:

• Work related notes (sick notes, back to work notes)

• Insurance notes for massage, physiotherapy, chiropractor, etc.

• Ear syringing

• Prescription renewals without a visit

• Sending notes, requisitions, prescriptions securely through email

• Copies of test results and consult reports

• Guarantor documents (i.e., for passports, birth certificates)

• Waiving of fee for missed appointments not cancelled within 24 hours

• Travel medicine advice and counselling

• Functional Abilities Forms

• Administration of vaccinations for immunizations not covered under OHIP

• Medical certificate for EI (Employment Insurance)

• Attending Physician’s Statement forms

• School or Work forms to verify Immunization Status

• Camp and School physical

• Discounted fee on Disability Tax Credit forms

• Rebooking of missed specialist appointment

• Replacement of lost/misplaced prescriptions or requisitions for tests

• Discounted fee on Driver’s Medical examination and form

• Letter for exclusion from jury duty

These services are otherwise still available for a flat fee, when requested.
